The Pharmacist Health Coaching – Cardiovascular Program is available to patients in Ontario, BC, Saskatchewan, Nunavut, Yukon and Northwest Territories, who have both high blood pressure and high cholesterol; who have extended health coverage with Green Shield Canada; and who are under 65 years of age. Pharmacists will determine patient eligibility based on a review of the patient’s medication history, in many cases at the time of dispensing.
This program will help demonstrate the benefits of pharmacist interventions in managing high blood pressure and high cholesterol, and will be of significant benefit to your patients and your business.
Completion of an online training program is a requirement for pharmacists’ enrollment and participation in the program.
