Family Health Teams

One of the key priorities for the Ontario government’s health care transformation agenda was the creation of Family Health Teams (FHTs).

Family Health Teams offer a supportive interdisciplinary environment that fosters the delivery of seamless comprehensive patient-centred primary health care.

The Ontario Pharmacists Association has always been supportive of this initiative, which provides pharmacists with opportunities to showcase our expertise as medication specialists. FHTs allow pharmacists to collaborate with other health care providers within the team in order to determine which pharmacist services will best meet each patient’s’ needs. This might include medication management, medication adherence, or chronic disease management programs delivered at the community pharmacy where patients’ typically access pharmacy services. 

Support resources for pharmacists

The Ontario Pharmacists Association has developed a Family Health Team resource guide to aid practitioners in the effective management of community medication issues. The guide recommends efficient and effective roles for FHT pharmacists and supporting community pharmacists.
