How does AD&D insurance protect me?

The Ontario Pharmacists Association’s AD&D plan is voluntary insurance that covers you 24 hours a day, seven days a week, both on and off the job. 

The AD&D plan is available for individual applicants, and family coverage is available for the applicant and his or her spouse and children.

Who is eligible for this insurance?

AD&D insurance is one of the easiest types of coverage to obtain — coverage is not underwritten and no medical evidence is required.

Coverage under OPA’s AD&D plan is available to:

  • members under age 70 who work a minimum of 20 hours a week on a regular full-time basis
  • members’ non-pharmacist/non-technician employees who are under age 70 and work a minimum of 20 hours per week on a regular full-time basis.

Who manages the AD&D insurance program?

The Ontario Pharmacists Association’s AD&D insurance program is managed by Crain & Schooley Financial, one of Ontario’s largest insurance brokerage and risk management firms. Crain & Schooley has been in the business since 1954 and has been providing service to OPA and its members since 1985.

Who underwrites the AD&D program?

The Ontario Pharmacists Association’s AD&D program is underwritten by Desjardins Financial Security, a member of the Desjardins-Laurentian Life Group (DLLG), a holding company that consolidates all the life and health insurance activities of the Mouvement Desjardins. As a partner in DLLG’s Group Network, Desjardins Financial Security is the seventh largest group insurer in Canada.

Who administers the program?

The program is administered by the Ontario Pharmacists Association's insurance department, which provides personal customer service dedicated to meeting your needs.

The program is overseen by the Ontario Pharmacists Association's insurance committee.

How do I apply for OPA’s AD&D insurance?

Download the application for insurance, and complete the form:

  • Section 1: Complete in full 
  • Section 2: Complete 2.1 and 2.2
  • Section 8: Sign and return
  • Section 9: To be retained by the proposed insured

Non-pharmacist/non-technician employees must have their application signed by the owner/sponsoring pharmacist. 

Mail the original copy of your completed application to:

Faxed, scanned, or photocopied applications will not be accepted. 

Do not send premium payments with your application. An invoice will be mailed with your coverage documents.

Within five business days of receipt of your application, the Association will mail you the certificate of insurance, details of the coverage, and an invoice for the initial premium.

When does coverage take effect?

Coverage takes effect on the first day of the month following receipt of your application. 

What forms of payment do you accept?

You may pay premiums by cheque or money order, payable to the Ontario Pharmacists' Association.
