Long-term care pharmacist

As a long-term care (LTC) pharmacist I not only provide clinical medication reviews for a complex population of patients, but I am also able to work closely with a team of healthcare professionals, including physicians, nurses, physiotherapists, and dietitians. My role is unique in that I have to go beyond following guidelines for how to treat conditions and disease states to provide the best possible care. These patients may react more to medications, experience more side effects, and may not benefit from medications the way younger patients do, so we aim to treat the patient - not their list of diagnoses - to ensure their time spent in a LTC home is as pleasant as possible.
