Unit Summary

Hamilton Health Sciences Pharmacy Services is embarking on an exciting, transformational journey of redefining patient care as we leverage the most advanced technologies to enhance the provision of highly reliable, safe, and quality care.  We are a multi-site department offering a wide range of specialized and complex services within inpatient, ambulatory,   retail pharmacy services in addition to a residency program. We have pharmacy facilities at the Juravinski Hospital and Cancer Centre, Hamilton General Hospital, McMaster University Medical Centre, St. Peter’s and West Lincoln Hospitals, and other sites as required including our Urgent Care Center and Alternate Health Facility. The scope of Pharmacy services also includes three retail pharmacies (Cancer Centre Pharmacy, McMaster Drugstore and General Drugstore).  We are an academic, tertiary care center offering a wide range of specialty patient services ranging from neonatal, pediatric, adult, geriatric and palliative care and are collaborators for clinical trials and research.  Our services are offered with recently renovated state-of-the-art cleanroom facilities for sterile and non-sterile compounding.

Position Summary

Join our growing pharmacy team at Hamilton Health Sciences in providing safe and effective medication while improving patient care and outcomes. 
At Hamilton Health Sciences, our pharmacy department plays an essential role in ensuring the effective distribution of medications and delivery of safe, high quality patient care to patients which facilitates successful treatment outcomes in compliance with legal, regulatory and accreditation standards.  The management and distribution of our medications is important in ensuring our patients consistently receive the excellent care for which we are known.  We are growing our pharmacy team and presently seeking a pharmacist to support us.  

The pharmacist is a key member of the interdisciplinary healthcare team whose key responsibilities include reviewing and assessing medication orders and collaborating with the patient and healthcare team directly to optimize medication use, and to identify and avoid medication-related problems to achieve desired patient outcomes. The pharmacist will provide drug information, promote evidence-based/cost effective drug therapy and be involved in educational activities for students, families, and staff. Ongoing collaboration and communication within the healthcare team and with the pharmacy team is essential.

Duties and responsibilities for the Overnight pharmacist will include supporting Hamilton General, Juravinski, St. Peter’s and West Lincoln Hospitals in addition to the Alternate Health Facility:

  • Review medication orders for safety and appropriateness, and verify orders in a fully electronic computerized prescriber order entry health information system
  • Provide drug information and clinical overnight pharmacy support (e.g. pharmacokinetic monitoring, etc.) 
  • Responsible for the oversight of drug distribution services in collaboration with pharmacy staff
  • Effectively communicate with nursing, medical staff, pharmacy team members and management
  • Other duties as assigned

Schedule Work Hours

Overnights, including weekends and holidays

Qualifications

  • Bachelor of Science in Pharmacy required.  PharmD is an asset.
  • Current registration with the Ontario College of Pharmacists required – Part A of Register
  • Previous hospital experience in unit dose distribution and/or IV admixtures preferred
  • Hospital residency program or equivalent an asset
  • Demonstrated ability to use strong interpersonal skills and to work as part of  as a member of a team 
  • Demonstrated ability to independently direct, organize and prioritize a consistent and varied workload
  • Ability to direct and teach staff.
  • Demonstrated written and verbal communication skills required to effectively collaborate with patients, peers, and other health care providers.
  • Demonstrated ability to identify, prevent and resolve drug related problems.
  • Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset.
  • Good understanding of change management; must be able to identify change needs and proactively act on those needs.
  • Leadership, problem solving, organization skills required.
  • Effective negotiation and conflict management resolution skills required.
  • Excellent attendance and discipline free record required.

Proficiency in both Official Languages would be an asset.
